- LSI application
- Copy of passport
- Copy of current visa
- Copy of current I-94
- Bank statement (minimum of $8,100 for 6 months of study or $16,200 for 1 year of study)
- Proof of highest education attended
- Payment:
- Processing fee: $250 (non-refundable)
